 Instructions:

• If you have participated in the Spring York lacrosse Program 2010, or in the Elmhurst Lacrosse Youth Spring League 2010 (8th graders), we have your paperwork on file and only need the information in the document below unless your information has changed:

1.) Registration Form for returning players (download form)

• If you have not participated in the York Programs, high school or youth (8th graders), or your information has changed, Please fill out the following 2 documents:

1.) REGISTRATION FORM FOR NEW PLAYERS (download form)

2.) LAX WAIVER (download form)
